Waiver of Liability and Assumption of Risk
(Must Read Before Enrollment)
By enrolling in a class or entering the facility of North St. Louis County Obedience Training Club, Inc. (NCOTC), I acknowledge and agree to the following:
1. Assumption of Risk
I understand that participation in dog training activities involves inherent risks, including but not limited to:
- Injury to myself, my dog, family members, or guests
- Dog bites, scratches, or other injuries from interaction with dogs or equipment
I voluntarily assume all risks associated with participation in training classes and use of the facility.
2. Release of Liability
I hereby release, waive, and discharge NCOTC, its officers, instructors, agents, and volunteers from any and all liability for injury, loss, or damage to persons or property, including injury to my dog, arising out of or related to participation in classes or use of the premises, including claims based on negligence of NCOTC or its representatives.
3. Indemnification
I agree to indemnify and hold harmless NCOTC and its representatives from any claims, demands, damages, or costs (including legal fees) arising from:
- My actions or omissions
- My dog’s behavior
- Any person or animal accompanying me
4. Responsibility for Dog
I agree that:
- My dog is under my control at all times
- I am solely responsible for my dog’s behavior and any damage or injury caused
- I will follow all instructor directions and facility rules
5. Health & Vaccination Requirements
I certify that my dog meets required vaccination standards (DHLPP, Rabies if age-appropriate, Bordetella) and is in good health to participate.
6. Aggressive Dog Policy
I understand that dogs displaying aggressive behavior may be dismissed from class at the discretion of the trainer, without refund.
7. Veterinary Care
If my dog is injured or becomes ill, I am solely responsible for all veterinary care and associated costs.
8. Visitors
I understand that all adult visitors must sign a waiver before entering the facility.
9. Refund Policy
No refunds will be issued after the first week of class.
