Vaccinations for Class
The following vaccinations are required to enroll in classes: DHLPP, Rabies (if your dog is 4 months or older), and Bordetella. Proof of these vaccinations is required the first night of class. The Bordetella vaccination is not part of your pets shots, please be sure this vaccination is given to your dog prior to entering class, it protects your dog against Kennel Cough and is also required if you board your dog.

WAIVER OF LIABILITY *** IMPORTANT *** PLEASE READ
By entering this facility I hereby waive, release, and discharge any and all claims for personal injury and/or property damage which I may have or which I may subsequently have against North St. Louis County Obedience Training Club, Inc. (hereafter known as NCOTC), its officers and/or any person associated with NCOTC, as result of my use of the property. In addition, I hereby agree to indemnify and hold harmless NCOTC, its officers and/or any other person associated with NCOTC, from any and all claims for personal injury, injury and/or property damage to any member of my family and/or other persons accompanying me to any training session or function on the property or surrounding area thereto, including costs of defense. I understand that my dog and I use the property at my own risk and that NCOTC, its officers and/or any person associated with NCOTC, are not liable for any accident and/or injury that may arise from use of the facility or said training. This release is intended to discharge NCOTC, its officers and/or any other person associated with NCOTC and their agents from and against any and all liability arising out of or in any way connected with such activities, even though that liability may arise allegedly or actually from any actions whatsoever of NCOTC or persons acting on its behalf.

NOTE—Any aggressive dogs are subject to dismissal at the discretion of trainer.

NOTE—Adult visitors (not enrollee on record) must sign a waiver of liability upon entering the building. During the COVID-19 pandemic, visitors are not allowed due to providing safe social distancing.

REFUNDS
No refunds will be issued after the 1st week of class.

You may also print the Enrollment Form instead of filling it out online and mail completed form with check. Call (314) 741-2445 for address. Online forms are processed quicker.
